Job summary

Total Care Therapy in Ohio is seeking an Occupational Therapist to evaluate residents and develop individualized treatment plans that improve independence in daily living activities, upper-extremity function, cognition, and safety.

You will work with an interdisciplinary team in assisted living and skilled nursing settings, with competitive compensation, mentorship, and opportunities for career advancement in a therapist-owned company.

Qualifications

• Current State OT License.
• Current CPR/BLS Certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
