Job summary

AccessHealth is seeking licensed Occupational Therapists to provide in-home and telehealth services to older adults, helping them improve mobility, independence, and quality of life.

You will evaluate patients, develop individualized treatment plans, document in EMR, and collaborate with a multidisciplinary team. Flexible options include full-time, part-time, and PRN.

Qualifications

• Active OT license or eligibility in practice state(s).
• Graduate from an accredited OT program.
• Current CPR certification.

Responsibilities

• Deliver one-on-one occupational therapy in private residences and senior living communities.
• Perform evaluations, develop individualized treatment plans, and provide evidence-based therapy sessions.
• Document care accurately within the EMR system and collaborate with an interdisciplinary team.

Physical Requirements Include

• Ability to exert moderate physical effort, assist with transfers, and perform therapeutic activities.
• Driving to patient locations and wearing PPE when necessary.
