Job summary

Enhance Rehabilitation in Belvidere, IL is seeking a qualified Occupational Therapist to join our SNF team. You will evaluate and treat patients, develop plans of care, set functional goals, and coordinate with facility staff to ensure high-quality care.

You will re-evaluate progress, adjust plans, document evaluations and progress notes, and supervise Occupational Therapy Assistants. A sign-on bonus is available. Join a dynamic team focused on patient outcomes and compassionate care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
