Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable functional goals.

You will collaborate with facility staff, educate patients and families, and document progress daily. Supervising Occupational Therapy Assistants is also part of the role, ensuring high-quality therapy services.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
