Enhance Rehabilitation in Wisconsin is seeking an Occupational Therapist to join our SNF team. You will evaluate and treat patients, develop plans of care, and work closely with facility staff to achieve functional goals.

As part of our team, you will provide patient and family education, document progress, and mentor therapy assistants as needed. This role emphasizes high-quality care and collaborative practice in a skilled-nursing setting.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
