Job summary

Enhance Rehabilitation seeks an Occupational Therapist to join a growing SNF team. You will evaluate and treat patients, develop plans of care, and work with facility staff to ensure high-quality care. You will also re-evaluate progress, adjust goals, and complete timely documentation. Supervising OT Assistants is part of the role.

Sign on bonus is offered. Milwaukee-area SNF setting with dedicated patient care focus and collaborative team environment.

Qualifications

• OT licensure required and in good standing.
• Experience in SNF setting preferred.

Responsibilities

• Evaluate and treat patients in SNF following individualized plan of care.
• Collaborate with facility staff to ensure excellent patient care.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.

Enhance Rehabilitation is currently seeking qualified Occupational Therapist candidates to join our dynamic team. As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
