Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our growing team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set measurable functional goals as part of the individualized care approach.

You will re-evaluate progress, adjust plans, provide education to patients and families, and complete timely documentation, while supervising Occupational Therapy Assistants and coordinating with facility staff to ensure outstanding patient

Qualifications

• Evaluations, re-evaluations and discharge planning.
• Documentation of patient progress and treatment plans.

Responsibilities

• Provide high-quality care to SNF patients.
• Evaluate and treat patients following individualized plans of care.
• Develop and revise plans of care and set functional goals.
• Educate patients and families about injury, progress and outcomes.
• Re-evaluate patient progress and adjust goals as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.
• Collaborate with facility staff to maintain high standards of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
