Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and work closely with facility staff to ensure outstanding patient outcomes.

You will supervise Occupational Therapy Assistants and complete timely documentation while educating patients about their progress and expected outcomes. Collaboration is essential to deliver high-quality care.

• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
