Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high-quality care.

You will also supervise Occupational Therapy Assistants, document progress, and provide patient and family education on injuries and expected outcomes. Sign on bonus available.

Qualifications

• Qualified Occupational Therapist candidates
• Experience working in SNF or similar setting preferred.
• Ability to develop and follow individualized plans of care.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Provide excellent care to patients and customer service to patients and families.
• Collaborate with facility staff to ensure excellent patient care.
• Evaluate new patients, develop plans of care and set measurable, functional goals.
• Treat patients and educate them on injuries, progress, and outcomes.
• Re-evaluate patient progress and revise plans of care as needed.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
