Job summary

Enhance Rehabilitation in Illinois is seeking a qualified Occupational Therapist to join our dynamic team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and set measurable functional goals.

You will collaborate with facility staff, educate patients and families about progress, document outcomes, and supervise Occupational Therapy Assistants to ensure high-quality care.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable goals.
• Treating patients, addressing concerns and educating them on injury, progress and probable outcomes.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
