Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set measurable goals as part of a collaborative care approach.

You will provide education to patients and families, document progress and outcomes, and work with facility staff to maintain high standards of patient care. Supervising Occupational Therapy Assistants is included in this role to support a cohesive rehab

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes
• Supervising Occupational Therapy Assistants
