Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our dynamic SNF team. The Occupational Therapist will evaluate and treat patients in a skilled nursing facility setting, following an individualized plan of care to improve function and outcomes.

The role involves evaluating new patients, developing and implementing plans of care with measurable goals, and educating patients and families about progress and expected outcomes.

Responsibilities

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, addressing concerns and educating them on injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes
• Supervising Occupational Therapy Assistants
• Collaborate with facility staff to ensure excellent care for patients
• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
