Job summary

Total Care Therapy, a therapist-owned rehabilitation company, is seeking a compassionate Physical Therapist to join our growing team in Columbus, Ohio. You will evaluate residents, develop individualized plans of care, and deliver skilled PT interventions to improve mobility and independence.

You will also supervise PTAs, document in EMR, educate residents and staff, and collaborate with an interdisciplinary team to provide patient-centered care.

Qualifications

• Current State Physical Therapist (PT) License is required.
• Current CPR/BLS Certification is required.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
