Job summary

Brown University Health seeks a skilled RN to coordinate discharge planning for observation patients at The Miriam Hospital in Providence. You will work with medical staff to advance care plans while ensuring efficient use of resources and adherence to care standards.

The role requires leadership in case management, collaboration with utilization management, and ongoing education to improve patient flow and reduce readmission risk.

Qualifications

• Licensed RN in Rhode Island or via Nurse Licensure Compact.
• Bachelor’s degree preferred; experience may be considered.
• Certification in Case Management preferred or to be obtained within 1 year of hire.
• BLS certification required.

Responsibilities

• Partner with medical staff and case management to facilitate discharge planning for observation patients.
• Proactively identify high‑risk patients and assist discharge planning.
• Collaborate with utilization management to ensure effective communication between teams.
• Participate in daily observation rounds to develop and implement discharge plans.
• Provide clinical supervision and direction to Case Management staff as needed.
• Contribute to 24/7 coverage for clinical and scheduling issues.

Education

Registered Nurse licensure (RI or multi-state)

Bachelor's degree preferred

Case Management Certification preferred or to be obtained within 1 year

BLS Certification required
