Job summary

Qualifications

• High School diploma or GED required.
• Bachelor's degree preferred in Social Work or Human Services.
• Licensed Social Worker preferred; 1–3 years related work experience required.
• 2 years of related work experience without a related Bachelor's degree.
• Experience with computer systems including web-based applications and Microsoft Office apps (Outlook, Word, Excel, PowerPoint, Access).
• Strong written and verbal communication skills.
• Ability to work in a team and provide excellent customer service.

Responsibilities

• Identify and connect patients with community resources via a multifaceted service plan.
• Maintain a caseload with multiple patient contacts per day.
• Advocate for patients and document in medical records.
• Develop liaisons with community and governmental agencies.
• Provide staff training on resources and procedural updates.
• Document obstacles to obtaining services and communicate with supervisors and staff.
• Visit agencies to broaden knowledge and relationships.
