Job summary

Beth Israel Deaconess Medical Center is seeking a Community Healthcare Worker to assess resources and coordinate with social work and medical staff to connect patients and families with community resources. Qualifications

• High School diploma or GED required; Associate’s degree preferred in Human Services.
• Excellent verbal and written communication skills.
• Experience with computer systems, including web-based applications and Microsoft Office.

Responsibilities

• Determines resources to meet patients’ and families’ needs, develops a multifaceted service plan.
• Acts as case manager to coordinate care with medical providers.
• Maintains a caseload with many patient contacts per day.
• Documents and updates the online medical record; participates in team meetings.
