Job summary

La Clinica seeks a Community Resource Specialist at Birch Grove Health Center in Medford, OR to connect patients’ social determinants of health with community resources and guide navigation.

You will screen for SDOH needs, provide resources, assist access, and document activities in EPIC per APCM guidelines. Occasional outreach and collaboration with the Community Resource Team Lead are expected.

Qualifications

• High school diploma or GED.
• Must achieve Community Health Worker certification within 6 months of employment and maintain certification.
• Excellent interpersonal communication and problem-solving skills.
• Ability to intervene and promote reconciliation, compromise and positive outcomes in difficult interactions.

Responsibilities

• Screen/assess for social determinants of health (SDOH) needs and connect patients to resources within the community.
• Provide relevant community and/or health care resources, assisting patients in navigation of accessing those resources when indicated.
• Ensure the documentation of patient care in EPIC and follow workflows that meet patient engagement standards as outlined in the APCM program.
• CareSTEP : Online Portal Engagement.
• CareSTEP : Accessing Community Resource / Service.
• CareSTEP : Transportation Assistance.
• CareSTEP : Coordinating Care.
• Advocates for patients and teaches strategies for self-advocacy.
• Table at community events to provide information about La Clinica services.
• May meet a patient in the community to support accessing a needed resource or service.
• Provides updated information to the Community Resource Team Lead to ensure up-to-date resource information.
