Beth Israel Lahey Health is seeking a Community Healthcare Worker to assess community resources for patients and families. You will collaborate with social work and medical staff, provide orientation on resources, and support patients in achieving self-reliance.

The role requires coordinating with care teams, maintaining a caseload, and assisting with entitlements. Home visits and specialization in areas like trauma or family planning may be involved.

Competenze

• High School diploma or GED required; associate degree preferred.
• 1–3 years of related work experience required.
• Excellent verbal and written communication skills.
• Experience with computer systems, including web-based apps and MS Office.

Mansioni

• Determine resources to meet patients' needs and coordinate care.
• Maintain caseload with extensive patient contacts and document in the electronic record.
• Advocate for patients and assist with entitlements.
• Develop liaisons with community/state agencies to facilitate services.
• Conduct home visits and build expertise in areas like domestic violence, trauma, HIV and family planning.
