Responsibilities

• Identify and connect patients with community resources to meet complex needs.
• Maintain a caseload with multiple patient contacts daily, including documentation in records.
• Advocate for patients and assist with access to entitlement programs and services.
• Provide staff training on resources and update procedures as required by state/local agencies.
• Develop liaisons with community and governmental agencies to navigate complex systems.
