• Establish mission, strategic plan, and operational practices for the PCC.
• Lead performance, improvement, risk management, and cost containment initiatives.
• Oversee clinical programs and ensure resources for high-quality care.
• Build cross-functional partnerships to meet PCC goals.
• Direct care transitions and community health program planning.
• Provide guidance to staff and feedback to leaders for program improvements.
• Represent VUMC in meetings and external partnerships.
• Plan, implement, and evaluate programs to optimize outcomes and satisfaction.
