Rutgers, The State University of New Jersey, via Rutgers Biomedical and Health Sciences (RBHS), seeks a Nursing Care Coordinator in the Department of Medicine with NJMS in Newark to deliver nursing care and coordinate TB-related services.

You will work on-site, collaborating with the Lattimore Clinic Medical Director, TB Institute, and NJ Department of Health, following nursing standards and care plans.

Competenze

• Registered Nurse licensure in NJ is required.
• At least three years of outpatient experience.
• At least one year of clinical nursing experience with familiarity in EMR.

Mansioni

• Assess patients' health status per nursing standards.
• Apply core public health nursing functions related to TB care with nurse case management.
• Plan, implement and evaluate individualized care based on assessment and medical plan.
• Provide direct patient care or supervise health team members.
• Educate patients and families about health and care plans.
• Prepare patients for discharge and home health maintenance.
