Job summary

Martin Luther Care Center in Bloomington, MN is seeking a Nursing Assistant/Registered (CNA/NAR) to support residents in a Skilled Nursing Facility and Transitional Care. Responsibilities include personal cares, vitals, and EMR documentation, with evening shifts and every-other weekend and holiday rotations.

Requirements include active MN Nursing Assistant Registry eligibility or completion of Ebenezer's CNA pathway; preferred candidates bring compassion for geriatric care and strong

Qualifications

• Active as Nursing Assistant, Registered (NAR) on the MDH Registry by start date or through Ebenezer's Pre-CNA program with post-hire timeframe.

Responsibilities

• Assist residents with personal cares per care plan: bathing, oral hygiene, grooming, toileting, dressing, showers; respond to call lights.
• May check and record vitals per patient care model.
• Provide AM/PM/NOC cares, including peri care and catheter care.
• Turn, lift or transfer residents with/without assistance.
• Observe residents and report status clearly to staff.
• Chart/document in EMR system.
• Address resident and family concerns.
• Receive/give shift reports to staff.
• Maintain room cleanliness; report equipment issues; answer phones promptly.

Job Overview

Martin Luther Care Center is seeking a Nursing Assistant/Registered (CNA/NAR) to support our residents in our Skilled Nursing Facility (Transitional Care and Long term care)! This role provides hands on care and support services for our senior age patients at our spacious care center in Bloomington, MN.

This patient support position includes;

• 56 hours every two weeks
• Evening shift
• Includes every other weekend (Saturday/Sunday) and holiday rotations

• Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights
• Nursing Assistant may per community patient care model check and record vitals
• AM/PM/NOC cares; including peri care and catheter care
• May turn, lift or transfer residents with or without other persons/mechanical assistance
• Observes residents and reports status to appropriate staff in a clear and concise manner
• Chart or document in electronic medical record system
• Seeks to resolve resident and family concerns
• Receives and gives report to appropriate staff during various times in work shift
• Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner

Required

Active as Nursing Assistant, Registered (NAR) on the MDH Nursing Assistant Registry by start date - OR- Individual who is hired into Ebenezer's Nursing Assistant Course or Ebenezer's Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.

Preferred

Ability to work with a diverse and geriatric population

Communicate efficiently, with compassion and understanding for residents and coworkers

Experience working as a HHA, PCA, DSP, NA, Resid
