Ebenezer is seeking a Nursing Assistant/Registered (CNA/NAR) to support residents in the Ebenezer Skilled Nursing Facility (TCU) in Edina, MN. This role provides hands-on care and patient support for adult short-stay patients.

Responsibilities include assisting with personal care, recording vitals, and documenting in EMR. The position emphasizes compassionate communication, teamwork, and adherence to care plans. Growth opportunities exist within TCU and Assisted Living.

Qualifications

• Active MN Nursing Assistant registry is required at start date.
• If hired into the Ebenezer Nursing Assistant Training program, a 4-month grace period to be active on the MN Nursing Assistant registry is granted.

Responsibilities

• Assist residents with personal cares per individual care plan (bathing, hygiene, grooming, toileting, dressing, showers).
• Record vitals and chart in the electronic medical record system as required.
• Support AM/PM cares, peri care and catheter care.
• Assist with turns, transfers, and mobility as needed.
• Observe residents and report status clearly to staff.

Job Overview

Ebenezer is seeking a Nursing Assistant/Registered (CNA/NAR) to support our residents in our Ebenezer Skilled Nursing Facility (Transitional Care)! This role provides hands on care and patient support services for our adult short stay patients at our spacious care center TCU in Edina, MN.

Aurora on Franceis a new, innovative senior community attached to Fairview Southdale Hospital in the heart of Edina, MN. Located conveniently near Hwy. 62 and France Ave. and is on a bus line. Aurora has 136 units offering a full continuum of care including Transitional Care, Enhanced Care and Assisted Living/Memory Care. This patient support schedule includes;

• 48 hours in two weeks
• Day shift; 6AM - 2PM
• Every other weekend and holiday rotation

Block Schedule:

Week 1: Monday, Friday
Week 2: Monday, Friday, Saturday, Sunday

Responsibilities:

• Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights
• Nursing Assistant may per community patient care model check and record vitals
• AM/PM cares; including peri care and catheter care
• May turn, lift or transfer residents with or without other persons/mechanical assistance
• Observes residents and reports status to appropriate staff in a clear and concise manner
• Chart or document in electronic medical record system
• Seeks to resolve resident and family concerns
• Receives and gives report to appropriate staff during various times in work shift
• Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner

Required:

• Active on MN Nursing Assistant registry by start date
• If hired into the Ebenezer Nursing Assistant Training program, granted a 4-month grace period from hire date to be active on the MN Nursing Assistant registry

Preferred:

• Ability to work with a diverse and geriatric population
• Communicate efficiently, with empathy and understanding for residents and coworkers
• Experience working as a HHA, PCA, DSP, NA, Resident Assistant
• Work experience in a customer service setting
