Job summary

Qualifications

• BSN preferred; 5+ years nursing experience strongly preferred, especially in ICU, ED, or Critical Care.
• Proficient in computer use, Microsoft Office, SharePoint, Windows, and Teams.
• Excellent communication, assessment, coordination, and patient education skills.
• Ability to respond effectively to urgent situations and work collaboratively in a team environment.

Responsibilities

• Perform patient assessments via phone, documenting symptoms and history.
• Coordinate emergency treatments and prioritize urgent calls.
• Provide patient education and facilitate scheduling.
• Support care coordination across departments and external providers.
• Train new personnel and ensure quality communication and documentation.
• Manage patient concerns, reinforce treatment plans, and ensure patient understanding.
• Suggest improvements to workflows and assist with external care coordination.
