Qualifications

• Graduate from a Specialty Training Program (required).
• Bachelor’s Degree in Nursing (preferred).
• Strong clinical support, coordination, and communication skills.
• Ability to collaborate within multidisciplinary teams and provide patient-centered care.

Responsibilities

• Coordinate pre- and post-hospital care and guide patients through care programs.
• Provide patient education to promote understanding of conditions and ongoing care.
• Support diagnosis and treatment by collaborating with healthcare providers.
• Communicate effectively with patients via mail, phone, and in person.
• Coordinate activities across interdisciplinary clinics and support services.
• Maintain follow-up systems to ensure timely surveillance and post-care monitoring.
• Assist with nursing duties and procedures, and educate team members.
