SWCHC, Inc. in Bridgeport, CT, is seeking an experienced nurse to join the Same Day Care Department. The role requires both LPN or RN licensure and the ability to work within outpatient, community-based settings.

The nurse will process patients, ensure continuity of care, and support data collection for grants while adhering to infection control and quality assurance protocols. This position involves 12-hour shifts, three days per week, including some weekends and holidays.

SUMMARY

The Nurse works collaboratively with providers and support staff to ensure the smooth and efficient running of the Same Day Care Department.

SPECIFIC DUTIES AND RESPONSIBILITIES

• Processes all patients according to the policies and guidelines established for Same Day Care.
• Ensures continuity of care is maintained in all aspects of patient care.
• Responsible for data collection systems and to report statistical data necessary for grant purposes as needed.
• Supports all quality assurance activities and infection control procedures according to established policies.
• Assists in maintaining stock inventory and ordering supplies, as necessary.
• Attends department, case management and other meetings as assigned.
• Maintains strict confidentiality at all times.
• Performs other duties and tasks as assigned.

Requirements

Qualifications

• Experienced and able to function effectively and professionally within an outpatient setting providing community-based care.

Education and/or Experience

• Graduate of an accredited nursing program
• CT Licensed Practical Nurse (LPN) license required or Registered Nurse (RN) license required

Physical/Work Environment Requirements:

• Variable 12-hour shifts (3 days a week) including weekends (Saturdays & Sundays) and holidays
• Minimal physical effort
• Must be able to operate computer and telephone continuously

In addition, this position will be eligible for additional benefits consistent with the role. The compensation of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation.
