Job summary

SWCHC, Inc. seeks an Internal Medicine Nurse to work with providers and staff to ensure smooth operation of the Internal Medicine Department in an outpatient setting.

You will process patients per guidelines, maintain continuity of care, collect data for grants, support quality assurance and infection control, manage inventory, attend meetings, and uphold confidentiality. Requires graduation from an accredited nursing program and CT LPN or RN license.

• Experienced in outpatient community-based care.
• Graduate of an accredited nursing program.
• CT LPN or RN license required.

SUMMARY

The Internal Medicine Nurse works collaboratively with providers and support staff to ensure the smooth and efficient running of the Internal Medicine Department.

SPECIFIC DUTIES AND RESPONSIBILITIES

• Processes all patients according to the policies and guidelines established for Internal Medicine.
• Ensures continuity of care is maintained in all aspects of patient care.
• Responsible for data collection systems and to report statistical data necessary for grant purposes as needed.
• Supports all quality assurance activities and infection control procedures according to established policies.
• Assists in maintaining stock inventory and ordering supplies, as necessary.
• Attends department, case management and other meetings as assigned.
• Maintains strict confidentiality at all times.
• Performs other duties and tasks as assigned.

Requirements

Qualifications

• Experienced and able to function effectively and professionally within an outpatient setting providing community-based care.

Education and/or Experience

• Graduate of an accredited nursing program
• CT Licensed Practical Nurse (LPN) license required or Registered Nurse (RN) license required
