Job summary

Optum is hiring Nurse Practitioners and Physician Assistants for our Senior Living program in Oregon. You will assess and manage high‑risk patients in a community‑based setting, coordinating care with interdisciplinary teams and reviewing plans to improve outcomes.

Qualifications

• Active license as Nurse Practitioner in the state of residence or ability to obtain by start date.
• Board certification in NP or PA track as applicable.
• Active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to gain collaborative practice agreement where required.
• Ability to perform physical requirements including lifting, stairs, and varied dwelling conditions.
• Reliable transportation to meet patient visits.

Responsibilities

• Conduct comprehensive patient assessments and manage acute or chronic condition episodes.
• Coordinate care through transitions across care settings with interdisciplinary team.
• Provide counseling and education to patients and caregivers.
• Document care accurately and in CMS-compliant manner.
• Engage with facility staff and families to implement care plans.
