Qualifications

• Active and unrestricted NP or PA license in the state of residence or ability to obtain by start date.
• Graduate of an accredited NP or PA program.
• Board certified in relevant specialty with recognized credential.

Responsibilities

• Conduct comprehensive assessments and manage acute or chronic conditions.
• Coordinate care with interdisciplinary team and care settings.
• Document findings concisely in CMS-compliant format.
• Engage in advanced illness and care planning discussions.
• Provide counseling and education to patients and caregivers.
• Support transition of patients across levels of care.
