Optum Senior Living in Oregon seeks a Nurse Practitioner or Physician Assistant to provide high-quality care in a Senior Living Program. You will perform comprehensive assessments, manage acute and chronic conditions, and partner with an interdisciplinary team to optimize patient outcomes.

Role emphasizes quality over volume with ~4–6 patient visits per day, no on-call requirement, and strong opportunities for professional growth within a national care network.

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
• Graduate of an accredited Physician Assistant degree program.
• Active and unrestricted NP/PA license in the state of practice (start date possible).
• Board certification as NP by AANP/ANCC or NCCPA certification for PA.

Responsibilities

• Conduct comprehensive assessments.
• Manage patients through acute events or exacerbations; include testing and interventions.
• Document accurately per CMS and ICD-10 requirements.
• Collaborate with interdisciplinary care team.
• Engage in advanced illness and care planning conversations.
• Educate patients and caregivers about care plans.
