Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities. You will perform direct patient care, manage chronic conditions, and coordinate with other providers to promote wellness and quality of life for seniors.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments, develop treatment plans, and provide ongoing care management for residents.
• Conduct regular rounds to assess residents’ conditions and adjust treatment plans.
• Collaborate with facility staff, family members, and other healthcare providers for coordinated care.
• Educate residents, families, and staff on disease prevention and chronic illness management.
• Maintain accurate documentation in the EMR and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care delivery.
