Job summary

Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing facilities and senior living communities. You will assess residents, manage chronic conditions, and coordinate care with a multidisciplinary team.

Qualifications

• Leadership and teamwork to coordinate care in multi-disciplinary settings.

Responsibilities

• Perform comprehensive patient assessments and develop treatment plans for SNF residents.
• Conduct routine rounds and monitor patient conditions with timely adjustments.
• Coordinate care with facility staff, families, and other providers.
• Educate residents, families, and staff on wellness and chronic disease management.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to enhance care and reduce readmissions.
