Job summary

Confident Search Consultants in the Greater Sacramento area is seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care within a community health center setting.

You will collaborate with physicians and multidisciplinary care teams to serve diverse, underserved populations, with relocation assistance available and a competitive compensation package ranging from $131,000 to $170,000 per year.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1–2 years of clinical experience preferred, especially in community health or underserved populations.
• Strong commitment to serving low-income, homeless, or medically underserved communities.
• Comfortable treating three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and therapeutic interventions within scope of practice
• Develop and implement treatment plans in collaboration with the Medical Director
• Monitor patient progress and modify care plans as appropriate
• Initiate emergency procedures when clinically indicated
• Provide preventive care, health education, and counseling services
• Maintain accurate and timely documentation in accordance with agency and regulatory standards
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care
