Job summary

Total Extended Care Services in Michigan is seeking a Nurse Practitioner or Physician Assistant to support our PM&R rehabilitation program in skilled nursing and sub-acute facilities.

This in-person role offers autonomy, collaborative team-based care, flexible scheduling, and no on-call requirements, with part-time opportunities and a focus on functional recovery and reducing readmissions.

Qualifications

• Active NP or PA certification required.
• Current Michigan licensure required.
• Strong communication and interdisciplinary collaboration skills.
• Ability to work independently while contributing to a team-based rehabilitation model.
• Interest in rehabilitation, senior care, pain management, and functional recovery.
• Reliable transportation for travel between skilled nursing facilities.

Responsibilities

• Partner with PM&R physicians to evaluate, manage, and support patients in sub-acute rehabilitation and skilled nursing facilities.
• Collaborate closely with therapy departments to help patients progress toward functional recovery goals.
• Coordinate care with primary care providers, therapists, nursing staff, social workers, and family members to ensure continuity and quality of care.
• Support on-site pain management and rehabilitation treatment plans designed to improve patient comfort and reduce hospital readmissions.
• Maintain timely and accurate documentation while balancing efficient patient care delivery.
• Contribute to a patient-centered, team-oriented culture focused on measurable rehabilitation outcomes and patient satisfaction.
