Job summary

Total Extended Care Services in Illinois is seeking a dedicated Nurse Practitioner or Physician Assistant to join a PM&R-focused team in sub-acute rehab facilities. You will partner with PM&R physicians and the therapy team to deliver high-quality rehab care and coordinate with primary care providers.

Role offers both full-time and part-time options, with an emphasis on timely charting and collaboration. In-person work at skilled nursing facilities, with a stable, team-oriented culture.

Qualifications

• Licensed Nurse Practitioner and/or Certified Nurse Practitioner per IL requirements.
• Licensed or Certified Physician Assistant as applicable in IL.
• Must hold IL licensure and be in good standing.
• Ability to communicate effectively with multiple disciplines in a rehab setting.
• Strong documentation and charting skills; work from home is possible for charting.

Responsibilities

• Partner with PM&R and therapy teams to deliver rehab care.
• Coordinate with patient’s primary care team for optimal outcomes.
• Timely charting/documentation; some charting can be done remotely.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
