Job summary

Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in the sub-acute rehab setting, working in person at skilled nursing facilities and partnering with a PM&R physician to improve patient outcomes.

In this role you will collaborate with therapy, nursing, and family teams to coordinate care, ensure timely charting, and support the goal of reducing readmissions while maintaining a patient-centered approach.

Qualifications

• Current IL NP or PA licensure with board certification.
• Experience in PM&R or sub-acute rehab is a plus.
• Ability to work in an interdisciplinary team and document care.

Responsibilities

• Collaborate with PM&R physician and rehab team to deliver rehab care.
• Coordinate with primary care, therapists, nurses, and families; document care timely.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
