Job summary

Total Extended Care Services is seeking skilled Nurse Practitioners or Physician Assistants to join our PM&R team in sub-acute rehab settings. This in-person role partners with PM&R physicians and therapy staff to enhance rehab outcomes for seniors across facilities.

Qualifications

• Licensed Nurse Practitioner (Required).
• Certified Nurse Practitioner (Required).
• Licensed Physician Assistant (Required).

• Must be licensed in MI.
• Reliable transportation.

Responsibilities

• Partner with the facility's therapy department to provide the best possible rehab care.
• Coordinate with the patient's primary care team to ensure optimal outcomes and timely charting/documentation (can be done from home).

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
