Job summary

Total Extended Care Services in Illinois is seeking a dedicated Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. The role collaborates with PM&R physicians and an interdisciplinary team to deliver high-quality rehab care in skilled nursing facilities.

You will partner with therapy and primary care teams, ensure timely charting, and accommodate flexible full-time or part-time schedules with no on-call requirements.

Qualifications

• Active IL license as NP/PA.
• Current NP/PA certification.
• Experience in PM&R or rehab setting preferred.

Responsibilities

• Partner with PM&R physician to deliver rehab care in sub-acute facilities.
• Collaborate with the patient’s primary care team to ensure optimal outcomes.
• Timely charting/documentation, can be done from home.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
