Total Extended Care Services, a growing Physical Medicine and Rehab group, seeks a Nurse Practitioner or Physician Assistant to join our team in sub-acute rehab settings.

Work alongside Board-certified PM&R physicians and a multidisciplinary team in skilled nursing facilities. You’ll be responsible for rehab-focused care, timely charting, and coordinating with primary care and therapy staff. Flexible full-time or part-time schedules; in IL, on-site roles.

Qualifikationen

• Licensed NP or PA in IL required or board-eligible.
• Certification and licensure in IL must be current.
• Experience in PM&R/sub-acute rehab is preferred.

Responsibilities

Partner with the facility's therapy department to provide the best possible rehab care. Communication and collaboration with the patient's primary care team to ensure the best outcomes. Timely charting/documentation is required but can be done from home.
