Job summary

Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab facilities. You will partner with our PM&R physician to deliver high-quality rehab care.

Role offers flexible scheduling with full-time and part-time options. You will collaborate with a multidisciplinary team of therapists, social workers, nurses, and families to improve patient outcomes and reduce readmissions. In-person care and coordination across facilities are key.

Qualifications

• Licensed NP and/or PA in IL is required.
• Experience in PM&R or sub-acute rehab is preferred.
• Ability to collaborate with therapists, social workers, nurses and families.

Responsibilities

• Partner with therapy department to deliver rehab care.
• Coordinate communication with patient’s primary care team.
• Maintain timely charting/documentation, including from home when appropriate.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting.
