$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program


This is a travel role covering the State of Connecticut and will require overnight travel. This role comes with great incentives!

As part of our Senior Community Care (SCC) team, you will provide care to patients at home in settings such as nursing homes and assisted living facilities, enhancing access to healthcare.

The SCC program is a comprehensive, coordinated care delivery model for members in facility settings. The APC in SCC provides care to high-risk members as part of an interdisciplinary team. This role offers work-life balance, with no on-call, weekends, or holidays. It requires travel across Connecticut.


Primary Responsibilities:

  • Provide clinical coverage during PTO or absences of primary APC
  • Manage acute medical situations or exacerbations of chronic conditions, including testing and interventions
  • Manage short-term, sub-acute rehabilitation episodes
  • Conduct comprehensive routine and acute care assessments
  • Manage medical and behavioral conditions effectively
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC with a national board certification:
    • For NPs: MSN from an accredited program and certification via AANP or ANCC, with preferred certifications in ANP, FNP, or GNP
    • For PAs: Degree from an accredited Physician Assistant program and NCCPA certification
  • Active, unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain post-hire
  • Ability to gain collaborative practice agreement if required
  • Physical ability to perform assessments and move equipment (e.g., 30-pound bag)
  • Reliable transportation
  • Minimum of 1 year of clinical experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of Geriatrics and Chronic Illness management
  • Proficiency in electronic medical records

**Please Note:** Employees must be in active status and commit to a minimum of 12 months in the role. The full student loan repayment benefit requires a 36-month role commitment within Home and Community. If leaving earlier, repayments cease.


The salary range is $89,800 to $176,700 annually, based on factors like experience, education, and location. Benefits include comprehensive coverage, incentives, stock options, and 401(k). All benefits are subject to eligibility.
