Nurse Practitioner / Physician Assistant - Connecticut State Traveler - Senior Care - Optum

$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the state of Connecticut and will require overnight travel. This role comes with great incentives!

The SCC program is a longitudinal, integrated care delivery initiative that coordinates clinical care for members in facility settings. The APC within SCC offers care to our highest-risk health plan members as part of an interdisciplinary team. This position offers work-life balance with no on-call, weekends, or holidays required. Travel throughout Connecticut is necessary.

Primary Responsibilities:

  • Provide clinical coverage for the primary APC during PTO or short/long-term absences
  • Manage members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Conduct comprehensive, routine, and/or acute care assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate and complete documentation, including ICD-10 coding
  • Collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC through a national board
    • For NPs: Graduate of an accredited MSN program and board certified by AANP or ANCC, with preferred certifications as ANP, FNP, or GNP
    • For PAs: Graduate of an accredited Physician Assistant program and NCCPA certified
  • Active and unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain post-hire, as per state regulations
  • Ability to gain a collaborative practice agreement, if applicable
  • Physical ability to perform assessments, including moving a 30-pound bag
  • Reliable transportation
  • At least 1 year of clinical experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients having complex behavioral, social, or functional needs
  • Knowledge of Geriatrics and Chronic Illness management
  • Proficiency in electronic medical records and documentation

**Please Note:** Employees must be in active status and remain in the role for at least 12 months. Leaving the role will terminate student loan repayment benefits. Full loan repayment requires a 36-month commitment within the role.

The salary range is $89,800 to $176,700 annually, based on full-time employment, experience, and other factors. Benefits include a comprehensive package, incentive programs, stock purchase options, and 401(k).
