The SCC program is a longitudinal, integrated care delivery initiative that coordinates clinical care for members in facility settings. The APC within SCC offers care to our highest-risk health plan members as part of an interdisciplinary team. This position offers work-life balance with no on-call, weekends, or holidays required. Travel throughout Connecticut is necessary.

**Please Note:** Employees must be in active status and remain in the role for at least 12 months. Leaving the role will terminate student loan repayment benefits. Full loan repayment requires a 36-month commitment within the role.
The salary range is $89,800 to $176,700 annually, based on full-time employment, experience, and other factors. Benefits include a comprehensive package, incentive programs, stock purchase options, and 401(k).
