A leading health care organization in Connecticut is seeking a Nurse Practitioner or Physician Assistant for a travel role supporting senior care. The SCC program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. The APC within SCC provides care to our highest-risk members and collaborates with an interdisciplinary team. This position offers work-life balance with no on-call, weekends, or holidays required, and involves travel across Connecticut.

**Please Note:** Employees must be in active regular status and remain in the role for at least 12 months from hire/rehire/transfer. Leaving the role will cease student loan repayments. Full loan repayment benefits require remaining in an APC or Physician role within Home and Community for 36 months.
The salary range is $89,800 to $176,700 annually, based on full-time employment and factors like location, experience, and certifications. UnitedHealth Group offers comprehensive benefits, incentives, stock purchase options, and 401k contributions. Benefits are subject to eligibility.
