Nurse Practitioner / Physician Assistant - Connecticut State Traveler - Senior Care - Optum

$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the state of Connecticut and will require overnight travel. This role offers great incentives!

As a team member of our Senior Community Care (SCC) product, we provide care to patients at home, in nursing homes, and assisted living facilities. This work enhances access and support for our patients.

The SCC program is a longitudinal, integrated care delivery model that coordinates clinical care for members in facility settings. The APC within SCC provides care to our highest-risk members and collaborates with an interdisciplinary team. This position offers work-life balance with no on-call, weekends, or holidays required, and involves travel across Connecticut.

Primary Responsibilities:

  • Provide clinical coverage for the primary APC during PTO or absences
  • Manage members through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Conduct comprehensive, routine, and acute care assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate and complete documentation, including ICD-10 codes
  • Communicate and collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC through a national board
    • For NPs: Graduate of an accredited MSN program, board certified by AANP or ANCC, with preferred certifications as ANP, FNP, or GNP
    • For PAs: Graduate of an accredited Physician Assistant program, board certified by NCCPA
  • Active and unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain it post-hire
  • Ability to gain a collaborative practice agreement if required by state
  • Physical ability to perform assessments and move a 30-pound bag
  • Reliable transportation
  • At least 1 year of clinical practice experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of geriatrics and chronic illnesses
  • Proficient in electronic and written medical documentation

**Please Note:** Employees must be in active regular status and remain in the role for at least 12 months from hire/rehire/transfer. Leaving the role will cease student loan repayments. Full loan repayment benefits require remaining in an APC or Physician role within Home and Community for 36 months.

The salary range is $89,800 to $176,700 annually, based on full-time employment and factors like location, experience, and certifications. UnitedHealth Group offers comprehensive benefits, incentives, stock purchase options, and 401k contributions. Benefits are subject to eligibility.
