Nurse Practitioner / Physician Assistant - Connecticut State Traveler - Senior Care - Optum

$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the state of Connecticut and will require overnight travel. This role comes with great incentives!

As part of our Senior Community Care (SCC) team, we provide care to patients at home in nursing homes and assisted living facilities, supporting access to care and improving patient outcomes.

The SCC program is an ongoing, integrated care delivery initiative that manages clinical care for members in facility settings. The APC within SCC provides care to high-risk health plan members as part of an interdisciplinary team. This role offers work-life balance with no on-call, weekends, or holidays required, and involves travel throughout Connecticut.

Primary Responsibilities:

  • Provide clinical coverage for the primary APC during PTO or absences
  • Manage members through acute medical issues or exacerbations of chronic conditions, including testing and interventions
  • Conduct comprehensive, routine, and acute care assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC through a national board
    • NPs: Graduate of an accredited MSN program, board certified by AANP or ANCC, with preferred certifications in adult-gerontology, adult/family, or gerontology nursing
    • PAs: Graduate of an accredited Physician Assistant program, certified by NCCPA
  • Active and unrestricted APC license in Connecticut
  • Current DEA licensure or ability to obtain post-hire
  • Ability to complete physical assessments and move a 30-pound bag
  • Reliable transportation
  • At least 1 year of clinical experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of geriatrics and chronic illnesses
  • Proficient in electronic medical records and documentation

Note: Employees must be in an active regular status and remain in the role for at least 12 months. Full student loan repayment benefits require a 36-month role commitment within Home and Community.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors such as location, experience, and certifications. Benefits include comprehensive health coverage, incentive programs, stock purchase options, and 401(k).
