The SCC program is an ongoing, integrated care delivery initiative that manages clinical care for members in facility settings. The APC within SCC provides care to high-risk health plan members as part of an interdisciplinary team. This role offers work-life balance with no on-call, weekends, or holidays required, and involves travel throughout Connecticut.

Note: Employees must be in an active regular status and remain in the role for at least 12 months. Full student loan repayment benefits require a 36-month role commitment within Home and Community.
The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors such as location, experience, and certifications. Benefits include comprehensive health coverage, incentive programs, stock purchase options, and 401(k).
