Nurse Practitioner / Physician Assistant - Connecticut State Traveler - Senior Care - Optum

$40,000 Student Loan Repayment Or $25,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program

This is a travel role covering the state of Connecticut and will require overnight travel. This role comes with great incentives!

As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home, in nursing homes, or assisted living facilities for seniors. This impactful work enhances access to care.

The SCC program is a longitudinal, integrated care delivery program that coordinates clinical care for members in facility settings. The APC within SCC provides care to our highest-risk health plan members as part of an interdisciplinary team. This position offers work-life balance with no on-call, weekends, or holidays required. Travel throughout Connecticut is necessary.

Primary Responsibilities:

  • Provide clinical coverage for the primary APC during PTO or absences
  • Manage members with acute medical issues or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Conduct comprehensive, routine, and acute care assessments
  • Manage acute and chronic medical and behavioral conditions effectively
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with the interdisciplinary care team

Required Qualifications:

  • Certified APC through a national board
    • NPs: Graduate of an accredited MSN program, board certified by AANP or ANCC, with preferred certifications in Adult-Gerontology Acute Care, Family, or Gerontology Nurse Practitioners
    • PAs: Graduate of an accredited Physician Assistant program, certified by NCCPA
  • Active, unrestricted APC license in Connecticut
  • Current DEA licensure/prescriptive authority or ability to obtain post-hire
  • Ability to gain a collaborative practice agreement if applicable
  • Physical ability to perform assessments, including moving a 30-pound bag
  • Reliable transportation
  • At least 1 year of clinical experience, preferably in long-term care

Preferred Qualifications:

  • Experience with patients with complex behavioral, social, or functional needs
  • Knowledge of Geriatrics and Chronic Illness management
  • Proficient in electronic medical records and documentation

**Please Note:** Employees must remain in active, regular status for at least 12 months, and in an APC or Physician role within Home and Community for 36 months to receive full student loan repayment benefits. If you leave earlier, repayments cease.

The salary range is $89,800 to $176,700 annually, based on full-time employment and factors such as location, experience, and certifications. Benefits include a comprehensive package, incentive programs, stock purchase options, and 401(k).
