Job summary

Total Extended Care Services is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab facilities in Illinois. You will partner with a PM&R physician and an interdisciplinary team to improve patient outcomes in skilled nursing environments.

This role offers flexible scheduling with full-time and part-time options and a focus on reducing readmissions. Responsibilities include collaborating with therapy teams, coordinating care with the patient's primary care

Qualifications

• Licensed NP, Certified NP, Licensed PA, Certified PA required.
• Must be licensed in IL.
• Reliable transportation required.

Responsibilities

• Partner with the facility's therapy department to provide rehab care.
• Communicate and collaborate with the patient's primary care team to ensure the best outcomes.
• Timely charting/documentation is required and can be done from home.

In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.
