Job summary

Total Extended Care Services in Cincinnati, OH, is seeking a Nurse Practitioner or Physician Assistant to join our PM&R team in sub-acute rehab settings. You will work with PM&R physicians and an interdisciplinary team to provide rehab care and coordinate with therapy and primary care teams.

Qualifications

• Must hold an OH licensure as a Nurse Practitioner or Physician Assistant.
• Eligibility for NP or PA certification in Ohio is required.

Responsibilities

• Partner with the facility’s therapy department to provide rehab care and coordinate with primary care teams.
• Maintain timely charting/documentation; some documentation can be done remotely.

Total Extended Care Services, a growing Physical Medicine and Rehab group is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.
