Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care to high-acuity patients across select markets in the United States. You will practice in the home setting with a patient-centered, team-based approach focused on quality outcomes.

Responsibilities include direct patient care, care coordination, and collaboration with other clinicians while maintaining compliance and documentation standards.

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Active CPR/BLS certification.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
