Job summary

HarmonyCares is seeking a licensed Nurse Practitioner to provide in-home primary care to high-acuity patients in New Jersey. The role emphasizes patient-centered care, not volume, with clinic-like support in the home and a Monday–Friday schedule.

Qualifications

• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide home health care to high acuity patients in their home, Monday - Friday.
• Take time to address the patient’s needs and coordinate care.
• Support appointments and travel with the patient in select markets.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
