Job summary

HarmonyCares is seeking Nurse Practitioners to deliver in-home primary care to high-acuity patients in their homes, Monday through Friday, across selected markets. The team uses a physician-driven model with support to ensure comprehensive, patient-centered care.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record
